What does OON mean?
The real answer
Under the No Surprises Act, in an emergency, at any ER, in your network or not, you pay your in-network share, figured on the recognized amount (under the federal rule the QPA, what the plan calls its median in-network rate for the service), and the doctor may not bill you the rest. It counts toward your in-network deductible and out-of-pocket maximum.

The fine print, in full
Standard Healthcare is a parody. No real company is depicted. The No Surprises Act: in an emergency, at any ER, a hospital's or a freestanding one, in your network or not, your share is your in-network share, figured on the recognized amount, not on the doctor's bill (45 CFR 149.110). It counts toward your in-network deductible and out-of-pocket maximum. The doctor may not bill you the rest (45 CFR 149.410). After you are stabilized the protection runs through the visit until you sign a valid consent, and you don't have to. Job-based, individual and federal-employee plans, for plan years from January 1, 2022; Medicare, Medicaid and TRICARE have rules of their own. Air ambulance is covered; ground ambulance is outside the federal law.
