What does out of network mean?
The real answer
Out of network means the doctor or hospital has no contract with your plan, so there is no agreed price: outside the cases the law protects, the plan may pay less or nothing, and the provider may bill you the difference. Under the No Surprises Act, in an emergency at any ER, or with an out-of-network clinician at an in-network hospital or surgery center, you pay only your in-network share (Public Health Service Act § 2799A-1, 42 U.S.C. § 300gg-111; 45 CFR 149.110 and 149.120), and the doctor may not bill you the rest unless, for some planned care, you signed a valid consent form in advance (42 U.S.C. §§ 300gg-131 and 300gg-132; 45 CFR 149.410 and 149.420).
