What happens after a claim is filed?

The real answer

An Explanation of Benefits is your plan’s statement of what it allowed and paid on a claim; the bill, if any, comes from the provider. For care the No Surprises Act protects, the provider may not bill you more than the in-network share the statement shows (42 U.S.C. §§ 300gg-131 and 300gg-132; 45 CFR 149.410 and 149.420).

What is an allowed amount? — answered from the law

Classic No. 10, the loud direct-response ad: mail in your claim, at no cost to us. It reads: “MAIL IN YOUR CLAIM TODAY!” “BY MAIL, AT NO COST TO US!” “WE ALREADY KNOW WHO YOU ARE! TELL US AGAIN! HAND YOUR HEALTH TO THE MAIL CARRIER! FAXES WELCOME! NO FINE FOR PAPER! IT’S SIMPLE! JUST BACKWARDS!”

Standard Healthcare is a parody. No real company is depicted, and no claim is accepted, by mail or otherwise. Every claim contains the information we already have on file; please send it again. Electronic submissions must first prove they are not a magazine or a bootlegger. A fine applies to each leaked data point of a person's health information, as we define a data point. No fine applies to paper. Postage is yours.
